This is a fully remote position, open to applicants in Philippines.
• Process and oversee member referrals received from the Marketing Department.
• Provide education to prospective members and their families regarding home care service requirements.
• Coordinate assessment procedures and assist with the enrollment process.
• Maintain continuous communication with members and families throughout the enrollment journey.
• Act as a liaison among members, nursing facilities, physician offices, hospitals, and insurance companies.
• Follow up on necessary documentation and enrollment prerequisites.
• Update daily reports and ensure accurate records are maintained in the system.
• Ensure that all member information is documented accurately and kept up to date.
• Minimum of 1 year of experience as an Intake Specialist, Home Care Intake Coordinator, or Long-Term Care Liaison.
• Proficient in both English and Spanish.
• Excellent customer service and communication abilities.
• Compassionate, professional, and focused on finding solutions.
• Strong skills in follow-up and organization.
• Detail-oriented with the ability to document accurately.
• Capable of managing multiple referrals and enrollment cases at once.
• Comfortable communicating with members, families, healthcare providers, and insurance companies.
