LVN Care Coordinator – Active California LVN License Required

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee and coordinate healthcare for members with short-term chronic care requirements within the boundaries of licensure.

• Execute, coordinate, and monitor care plans designed by providers throughout the care continuum.

• Facilitate and track member progress and services to ensure compliant and cost-effective care.

• Guarantee member access to services that align with their health requirements.

• Identify, evaluate, and manage members based on established criteria.

• Review and execute care plans while supplying educational materials to members.

• Collaborate with Primary Care Physicians, Nurse Practitioners, RN case managers, and specialists to ensure coordinated care.

• Support providers and case managers in resolving issues and challenges related to services.

• Assess the effectiveness of interventions to inform case management referrals and outcomes.

• Engage in the formulation of utilization/care management policies and procedures.

• Advise patients and families regarding available care options.

• Provide follow-up care and implement care plans as directed by Nurse Practitioners.

• Carry out responsibilities mainly through telephone communication.

• This position does not include supervisory duties.

⛳️ Requirements

• A minimum of 5 years of clinical case management experience, or a combination of education and experience that provides a comparable background.

• Possession of a current, unrestricted LVN license in the state of application.

• An associate degree is preferred.

• Capability to perform each essential duty effectively.

• Ability to fulfill the physical demands of the position, which includes frequently lifting and/or moving up to 10 pounds.
