LTSS Service Care Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Create, evaluate, and oversee comprehensive care management initiatives

• Design or assist in crafting individualized service care plans for long-term care clients

• Inform members and their families/caregivers about available services and benefits

• Assess member needs and resources, recommending or facilitating suitable plans

• Formulate ongoing long-term care strategies and identify appropriate providers, specialists, and community resources

• Coordinate efforts among members, families/caregivers, and care provider teams to guarantee service accessibility

• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care requirements

• Track care plans, member status, and outcomes, suggesting necessary updates

• Engage with long-term care healthcare providers and partners

• Gather, record, and uphold member information and care management activities to ensure regulatory compliance

• Conduct home and other site evaluations to assess needs and collaborate with healthcare providers and partners

• Deliver or arrange education on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits

• Provide leadership insights on opportunities to enhance care quality and cost-effective service delivery

• Carry out additional responsibilities as assigned

⛳️ Requirements

• Bachelor's degree

• 2–4 years of relevant experience

• Equivalent experience gained through achievements of applicable knowledge, duties, scope, and skills reflective of this position's level may be considered

• Candidate must be a resident of Pennsylvania

• Approximately 50% travel to nursing facilities required

• Previous experience in assessment is highly preferred

• Familiarity with Community Health Choices (CHC) is strongly preferred
