LTSS Service Care Coordinator

This is a fully remote position, open to applicants in Texas.

📋 Description

• Support the development, evaluation, and coordination of comprehensive care management initiatives.

• Create or assist in formulating individualized service care plans for members in long-term care.

• Inform members and their families/caregivers about the services and benefits available to them.

• Assess member requirements and existing resources, proposing or facilitating plans for the best possible outcomes.

• Establish continuous long-term care plans and identify necessary providers, specialists, and community resources.

• Facilitate communication between members, families, caregivers, and care provider teams to ensure service accessibility.

• Offer support for resources related to employment, housing, participant direction, independent living, justice, and foster care needs.

• Oversee care plans, member statuses, and outcomes, suggesting updates based on evolving member needs.

• Engage with healthcare providers and partners within the long-term care sector.

• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.

• May conduct on-site visits to evaluate member needs and collaborate with providers or relevant resources.

• Provide or facilitate instruction on procedures, healthcare provider guidelines, service options, referrals, and healthcare benefits.

• Offer leadership insights on opportunities for enhancing care quality and cost-effective service delivery.

• Undertake additional responsibilities as assigned.

⛳️ Requirements

• A Bachelor's degree along with 1 year of relevant experience, or equivalent experience gained through applicable knowledge, duties, scope, and skills.

• Must be a resident of Texas.

• Direct experience working with individuals with disabilities and/or vulnerable populations suffering from chronic or complex conditions, including children and young adults, within three of the last five years.

• Other state-specific criteria may be applicable.
