This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee incoming vital signs, symptoms, and health indicators for a group of patients enrolled in Cadence remote monitoring.

• Assess alerts and abnormal readings based on established clinical criteria to identify patients needing immediate clinical intervention.

• Reach out to patients with out-of-range readings to collect clinical information and verify context regarding symptoms, medications, and adherence.

• Collaborate with Cadence nurses and nurse practitioners to direct flagged patients to the suitable intervention and ensure follow-up is completed.

• Review AI-generated screening results and AI-prepared documentation for clinical accuracy prior to it being presented to a provider, making necessary corrections.

• Record every patient interaction in the clinical record and create care summaries for primary care providers and cardiologists.

⛳️ Requirements

• Active Licensed Practical/Vocational Nurse (LPN/LVN) license with compact licensure.

• At least 5 years of experience working as an LPN/LVN.

• A minimum of 2 years of experience in a cardiology environment.

• Background in remote patient monitoring or telemedicine, including EMR-based clinical documentation.

• Strong written and verbal communication skills for direct patient interactions and provider-facing documentation.

• Proficiency with AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms.

• Capability to engage credibly with providers and patients regarding the role of AI in care delivery and the areas where human judgment is essential.
