Licensed Practical/Vocational Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in compliance with the state Nurse Practice Act.

• Execute and adhere to personalized care plans set forth by the RN.

• Provide education to patients and their families regarding self-care practices and diagnosis-related information.

• Accurately document care provided in a timely manner.

• Work collaboratively with the care team, including OT, PT, HHA, and other disciplines, and relay patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, maintaining aseptic technique.

• Perform additional duties as necessary for patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Current LPN or LVN license in California.

• A minimum of 3 years of nursing experience, preferably in home health or med/surg settings.

• Active CPR certification from AHA.

• A valid driver's license, a reliable and insured vehicle, and the ability to travel to patient residences.

• Excellent communication skills and a collaborative, patient-centered approach.
