Licensed Practical Nurse, Patient Advocate

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide remote support to Medicare members throughout the entirety of their care journey.

• Utilize your LPN/LVN clinical expertise while becoming proficient in Solace’s unique platform, tools, and standards.

• Establish trusting relationships with patients through empathy and attentive listening.

• Evaluate patient situations, prioritize their needs, and formulate care plans that ensure continued progress.

• Create care plans that address both clinical and everyday challenges, such as access to food, transportation for appointments, and in-home assistance.

• Assist patients in navigating complex care coordination, resource access, and administrative hurdles.

• Offer non-clinical advocacy instead of direct medical care or advice.

• Interact with patients via video or phone, with no in-person meetings.

⛳️ Requirements

• Valid LPN or LVN license in good standing.

• Experience engaging with patients from various backgrounds and communities.

• Compassionate nature focused on caring for others and advocating for those in need of assistance.

• Capability to manage multiple priorities and maintain composure under pressure.

• Quick adaptability to learn new systems, platforms, and tools.

• Proactive ownership and the ability to independently recognize and address necessary tasks.

• Applicants must reside in the United States.

• Availability for some hours during core business times, Monday–Friday.

• Must possess authorization to work in the United States.

• Experience in creating care plans is not mandatory.
