Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in compliance with the state's Nurse Practice Act.

• Execute and adhere to personalized care plans developed by the RN.

• Provide education to patients and their families on self-care methods and information related to diagnoses.

• Accurately document care in a timely manner.

• Work collaboratively with the care team, including OT, PT, HHA, and other professionals, while communicating patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, maintaining aseptic techniques.

• Carry out additional duties as necessary for patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Active LPN or LVN license in the state of California.

• Over 3 years of nursing experience; preference for home health or med/surg backgrounds.

• Current CPR certification from the AHA.

• Valid driver's license, a reliable insured vehicle, and the capability to travel to patient residences.

• Excellent communication skills with a collaborative, patient-centered approach.
