Lead Nurse Practitioner – Remote Care Model

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Oversee and coordinate care for intricate patient demographics

• Operate with a significant degree of independence, including the ability to prescribe medications

• Collaborate effectively with physicians, care managers, and health insurance plans

• Promote proactive care management and early intervention strategies

• Guide patients during care transitions (from hospital to home)

• Deliver virtual care and advice to minimize unnecessary emergency room visits

⛳️ Requirements

• Must reside in Arizona

• 3–5+ years of clinical and leadership experience required

• Comfortable with independent practice

• Familiarity with telehealth or remote patient management

• Experience working with complex patient populations (Medicaid, elderly, behavioral health)

• Active Nurse Practitioner license in Arizona

• Master’s degree or higher educational attainment

• Excellent communication and care coordination abilities
