Integrated Care Assistant II – National IKC

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide administrative assistance to the DaVita Integrated Kidney Care Nurses and other members of the Integrated Care National Team.

• Gather, verify, input, and maintain patient, provider, payer contact, and referral information within the medical management database.

• Create reports and other materials for distribution.

• Communicate with hospitals and monitor patient admissions and discharges.

• Transcribe lab results and coordinate with providers to confirm services.

• Monitor reports and data feeds, engaging with the Integrated Care Team to ensure data accuracy and completeness.

• Participate in team meetings, conferences, and training sessions as necessary.

• Adhere to teammate guidelines, employment policies, and departmental or company procedures.

• Perform additional duties and responsibilities as assigned.

⛳️ Requirements

• A High School diploma or equivalent is required.

• Prior experience in an administrative role is essential.

• Proficiency in using MS Office applications and other databases is required.

• Previous remote work experience is a plus.

• Ability to handle confidential information with discretion and sound judgment.

• Professional verbal communication skills and telephone etiquette.

• Basic proficiency with the Medical Information System Technology (MIST) is required within 60 days of employment or position change.

• Consistent, regular, and punctual attendance as scheduled is expected.

• Availability to work remotely Monday–Friday, starting between 7 and 7:30 am CST.

• Capability to thrive in a fast-paced environment with minimal supervision.

• Strong multi-tasking skills are necessary.
