This is a fully remote position, open to applicants in Pennsylvania.
• Act as the main contact for patients throughout the intake procedure
• Respond promptly to patient inquiries and offer professional support
• Clarify subsequent steps, collect necessary information, and connect patients with the right care teams
• Manage both inbound and outbound communications with patients, healthcare providers, hospitals, physician offices, and medical facilities concerning referrals and care coordination
• Collect, assess, and accurately input referral details into the company's systems
• Receive and assess referral packets, medical orders, and accompanying documentation
• Organize home health services according to patient requirements, geographical location, and insurance specifications
• Confirm treatment timelines, first-dose administration, and needs for home infusion scheduling
• Upload and keep precise patient records and documentation in the medical record system
• Collaborate with Intake Coordinators and nurses to efficiently process referrals through intake
• Communicate updates regarding referrals and pending items to avoid delays in care
• Accurately transfer insurance details to the Insurance Verification teams
• Uphold patient confidentiality while providing outstanding service
• Assist departmental operations through administrative tasks, phone support, and document management
• Contribute to a collaborative, quality-focused, accurate, and patient-centered atmosphere
• High school diploma or GED equivalent
• At least 1 year of experience in a healthcare setting, medical office, pharmacy, home health, or related healthcare support role
• Familiarity with medical terminology
• Strong verbal and written communication abilities
• Capability to juggle multiple priorities in a fast-paced setting while maintaining attention to detail
• Proficient in Microsoft Office applications, including Outlook, Word, and Excel
• Excellent organizational skills and a dedication to delivering exceptional customer service
• Ability to safeguard confidentiality and manage sensitive patient information appropriately
• Preferred experience in supporting referral management, patient intake, home infusion, specialty pharmacy, or care coordination workflows
• Preferred experience with electronic medical records or healthcare management systems
• For remote work, a cable broadband or fiber optic internet connection with a minimum of 10 Mbps download/5 Mbps upload
