This is a fully remote position, open to applicants in Florida, +6 more states.
β’ Act as the main contact for patients and offer prompt, professional assistance throughout the intake process.
β’ Assist patients in comprehending subsequent steps, collecting necessary information, and connecting them with the relevant care teams.
β’ Manage both inbound and outbound communications with patients, providers, hospitals, physician offices, and healthcare facilities regarding referrals and care coordination.
β’ Collect, review, and accurately input referral information into company systems.
β’ Receive and triage referral packets, medical orders, and associated documentation.
β’ Organize home health services based on patient requirements, geographic location, and insurance specifications.
β’ Verify treatment timelines, including the administration of first doses and scheduling for home infusion needs.
β’ Upload and maintain precise patient records and documentation in the medical record system.
β’ Collaborate with Intake Coordinators and nurses to efficiently process referrals through the intake workflow.
β’ Communicate updates on referrals and outstanding items to relevant team members.
β’ Accurately transfer insurance information to Insurance Verification teams.
β’ Provide outstanding service while ensuring patient confidentiality is upheld.
β’ Assist departmental operations with administrative tasks, phone support, and document distribution.
β’ Contribute to a cooperative environment that emphasizes quality, accuracy, and patient-centered care.
β’ High school diploma or GED.
β’ At least 1 year of experience in a healthcare, medical office, pharmacy, home health, or a similar healthcare support setting.
β’ Familiarity with medical terminology.
β’ Excellent verbal and written communication skills.
β’ Capability to handle multiple priorities in a fast-paced environment while retaining attention to detail.
β’ Proficient in Microsoft Office applications, including Outlook, Word, and Excel.
β’ Strong organizational skills and a dedication to providing outstanding customer service.
β’ Ability to maintain confidentiality and manage sensitive patient information appropriately.
β’ Preferred: experience in supporting referral management, patient intake, home infusion, specialty pharmacy, or care coordination workflows.
β’ Preferred: experience with electronic medical records or healthcare management systems.
β’ Home internet access via cable broadband or fiber optic service with a minimum of 10 Mbps download and 5 Mbps upload speed.
