Inpatient/Outpatient Coder

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct remote reviews of VA health records, accurately coding both inpatient episodes and outpatient encounters for VISN 7.

• Identify and report on diagnoses, procedures, services, conditions, and essential data elements.

• Assign and sequence ICD-10-CM, ICD-10-PCS, CPT, and HCPCS Level II codes following official guidelines and VA standards.

• Code outpatient clinic visits, urgent-care services, radiology, laboratory, ancillary services, and ambulatory surgical services as designated.

• Code inpatient episodes, including primary and secondary diagnoses, procedures, present-on-admission indicators, and DRG-related data.

• Complete Patient Treatment File transition types and adhere to facility protocols for the opening, correction, and retransmission of inpatient records.

• Implement Coding Clinic guidance, CPT instructions, Correct Coding Initiative bundling rules, modifiers, and facility-specific protocols.

• Review documentation for any missing, conflicting, incomplete, duplicate, erroneous, or insufficiently supported information.

• Abstract and input or validate data in CPRS, VistA, approved encoder systems, and other authorized VA applications.

• Re-evaluate coded encounters during pre-bill reviews or post-denials; make necessary corrections or provide coding justification.

• Maintain a minimum coding accuracy of 95 percent and comply with facility turnaround and delivery expectations.

• Communicate any documentation deficiencies, coding inquiries, and workflow issues through established channels.

• Safeguard VA information and complete all required privacy, security, training, quality-review, and corrective-education activities.

⛳️ Requirements

• U.S. citizenship is mandatory.

• Proficiency in English; must be able to read, write, speak, and understand the language.

• A minimum of two years of coding experience in the designated coding area.

• Formal education in anatomy and physiology, medical terminology, pathology and disease processes, pharmacology, health-record content, reimbursement methods, and current coding systems.

• Active RHIA, RHIT, CCS, or CCS-P credential from AHIMA, or an active CPC or CPC-H credential from AAPC.

• Proficient in ICD-10-CM, ICD-10-PCS where applicable, CPT, HCPCS Level II, coding guidelines, encoder tools, electronic health records, CPRS, and VistA-related data entry.

• At least 95 percent coding accuracy is required.

• Capability to meet assigned turnaround and quality benchmarks.

• Successful completion of the Low Risk NACI background check.

• Completion of all necessary VA privacy, information-security, mandatory, orientation, annual, quality-review, and corrective-education training.

• Ability to work securely and independently from an approved location within the United States while safeguarding VA information.

• Must maintain required credentials, access approvals, and knowledge of inpatient and outpatient coding throughout the assignment.

• Required submission of an up-to-date resume, signed Letter of Intent, completed candidate cover page, proof of active accepted AHIMA or AAPC credential, educational or training documents, OIG exclusion search results, and any requested background, security, identification, privacy, training, or onboarding documents.
