This is a fully remote position, open to applicants in New Hampshire.
• Conduct In-Home Health Evaluations at members' residences.
• Interact with individuals on a personal level to address health inquiries.
• Assess medications and review medical history.
• Carry out physical evaluations.
• Execute additional diagnostic tests, such as diabetic eye exams, spirometry, and bone density screenings when prescribed.
• Utilize an iPad and follow clinical workflows for each evaluation.
• Perform virtual visits as necessary.
• Offer Signify Health services, including Diagnostic & Preventive Services.
• Travel locally and within licensed states as required.
• Foster trusted relationships to enhance health outcomes.
• Assist individuals in accessing appropriate care and identifying care gaps.
• Relay findings and referrals to health plans and primary care physicians as needed.
• Hold an active, unrestricted license(s) in the designated coverage area(s).
• Capable of obtaining and maintaining multi-state licenses in the coverage area, as necessary.
• Must possess an MD or DO qualification.
• Willingness to travel locally and within licensed states.
• Ability to travel up to 100% of the time within a specified coverage area.
• Competence to safely perform duties in homes that may include household pets, secondhand smoke, and airborne allergens or irritants.
• Capability to conduct virtual visits as needed.
• Proficient in performing medication and medical history reviews, physical evaluations, and additional diagnostic tests when required.
• Anticipated to work 40 hours per week.
