In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Vermont.

📋 Description

• Conduct in-home health evaluations at members' residences

• Travel within a designated coverage area, which may involve overnight accommodations, car rentals, and/or flights

• Establish trusting relationships and contribute to enhancing health outcomes

• Perform visits lasting approximately 45 minutes

• Review patients' medications and medical histories

• Execute physical evaluations

• Carry out additional diagnostic tests as instructed, such as diabetic eye exams, spirometry, and bone density screenings

• Utilize an iPad and follow clinical workflow for each evaluation

• Address health-related inquiries and provide health education

• Conduct virtual visits when necessary

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services products

• Serve all counties within the state and adapt to other areas based on business requirements

⛳️ Requirements

• Possess an active, unrestricted license(s) in applicable coverage area(s)

• Hold board certification (mandatory)

• Capable of obtaining and maintaining multi-state licenses in the coverage area as needed

• Able to cover all counties within the state and adjust to other areas based on business needs

• Comfortable and capable of safely performing duties in homes with pets, secondhand smoke, and potential airborne allergens or irritants

• Qualifications as a Nurse Practitioner or Physician Assistant

• Willingness to travel locally and within licensed states

• Availability to travel up to 100% of the time

• Competence in conducting virtual visits as required

• Proficiency in using an iPad and clinical workflow

• Commitment to working 40 hours per week
