In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Nevada.

📋 Description

• Conduct In-Home Health Assessments in the residences of members.

• Establish rapport with members to foster trusted relationships aimed at enhancing health outcomes.

• Review patient medications and medical histories.

• Execute physical evaluations.

• Carry out additional diagnostic tests as requested by the member’s health plan, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Address health-related inquiries and provide health education.

• Travel locally and across licensed states, potentially up to 100% of the time.

• Conduct virtual consultations as required.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

• Assist in connecting members with appropriate care and addressing gaps in care.

⛳️ Requirements

• Active, unrestricted license(s) in the respective coverage area(s).

• Board certification is mandatory.

• Capable of obtaining and maintaining multi-state licenses in the coverage area as needed.

• Must be able to cover all counties within the state and adapt to other business needs.

• Comfortable working in homes that may have household pets, secondhand smoke, airborne allergens, or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Ability to travel locally and within licensed states.

• Willingness to travel up to 100% of the time, including potential overnight hotel stays, car rentals, and/or flights.

• Capability to conduct virtual visits as necessary.

• Proficient in using an iPad and following clinical workflows.

• Expected weekly hours: 40.
