In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Idaho.

📋 Description

• Conduct in-home health evaluations within members' residences.

• Interact with individuals to help bridge care gaps.

• Perform visits lasting approximately 45 minutes, which include reviews of medication, medical history, and physical evaluations.

• Execute additional diagnostic tests as directed by the individual's health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflow for each assessment.

• Establish trusted relationships to enhance health outcomes.

• Travel nationwide to connect individuals with appropriate care.

• Meet the requirements for the designated clinician role with Signify Health.

• Travel locally and within licensed states, covering all counties as necessary.

• Conduct virtual visits when required.

• Offer Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screenings.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Active and unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Ability to acquire and maintain multi-state licenses in the coverage area as required.

• Capability to cover all counties within the state and adapt to other business needs.

• Comfort and safety in performing duties in homes that may have pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states.

• Readiness to travel up to 100% of the time, which may include overnight hotel stays, car rentals, and/or flights.

• Competence to conduct virtual visits as necessary.
