In-Home Physician – Per Diem

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Conduct approximately 45-minute In-Home Health Evaluations in the residences of members.

• Interact one-on-one with members and address their health-related inquiries.

• Assess medications and review medical history.

• Carry out physical evaluations.

• Execute additional diagnostic tests as prescribed by the member’s health plan, including diabetic eye exams and spirometry.

• Utilize an iPad and follow clinical workflow for each evaluation.

• Establish trustworthy relationships to enhance health outcomes.

• Travel nationwide to connect individuals with suitable care.

• Collaborate as a clinician colleague with Signify Health.

• Travel locally and within licensed states as required.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, which include Diagnostic & Preventive Services products.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Multi-state licenses are preferred.

• Must be able to cover primary locations including Colorado Springs and El Paso County.

• Must be comfortable and safe performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Must hold a physician qualification as either an MD or DO.
