In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in South Dakota.

📋 Description

• Conduct health evaluations in the homes of members.

• Interact with individuals to help close gaps in their care.

• Perform visits lasting approximately 45 minutes each.

• Review patients' medications and medical histories.

• Carry out physical evaluations.

• Administer additional diagnostic tests as ordered, which may include diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and clinical workflow for every evaluation.

• Establish trusted relationships to enhance health outcomes.

• Travel nationwide to connect individuals with appropriate care.

• Collaborate as a clinician colleague at Signify Health.

• Engage in local travel as well as travel within licensed states.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

• Travel may necessitate overnight hotel stays, car rentals, and/or flights.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Possession of active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Ability to acquire and maintain multi-state licenses in the designated coverage area, as required.

• Capable of covering all counties within the state and adaptable to other areas based on business needs.

• Must be comfortable and safe performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Primarily expected to travel up to 100% of the time across a defined coverage area.

• Anticipated weekly working hours: 40.
