In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Perform visits lasting around 45 minutes, which include reviews of medical history and medications as well as physical evaluations.

• Execute additional diagnostic tests as requested by the health plan, such as diabetic eye examinations and spirometry tests.

• Utilize an iPad and clinical workflow during each evaluation.

• Establish reliable relationships to enhance health outcomes.

• Travel across the nation to connect individuals with suitable care options.

• Travel locally and within licensed states as necessary.

• Conduct virtual consultations as required.

• Offer Signify Health services, encompassing Diagnostic & Preventive Services like spirometry, diabetic eye exams, and bone density screenings.

• Respond to health inquiries and provide education, without prescribing medications, ordering lab tests, or modifying current treatment plans.

⛳️ Requirements

• Possession of active, unrestricted licenses in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as necessary.

• Capability to conduct virtual consultations as needed.

• Ability to safely perform duties in homes that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
