In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct In-Home Health Evaluations at members’ residences

• Establish trusted relationships to enhance health outcomes

• Carry out visits lasting approximately 45 minutes

• Review patients' medications and medical history

• Execute physical evaluations

• Perform additional diagnostic tests as prescribed by the health plan, including diabetic eye exams and spirometry

• Utilize an iPad and clinical workflow for each evaluation

• Offer health education and respond to members’ inquiries

• Travel nationwide to connect individuals with appropriate care

• Travel locally and within licensed states as necessary

• Conduct virtual visits when required

• Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening

• Meet health plan members in their own homes

⛳️ Requirements

• Possession of an active, unrestricted license(s) in relevant coverage area(s)

• Board certification is mandatory

• Multi-state licenses are preferred

• Capability to cover Berks, Bucks, Chester, Delaware, Montgomery, or Philadelphia Counties

• Willingness to travel locally and within licensed states as needed

• Ability to conduct virtual visits when necessary

• Competence to perform duties safely in homes with household pets, secondhand smoke, and potential airborne allergens or irritants

• Qualifications as a Nurse Practitioner or Physician Assistant

• Availability for full-time work, with 40 anticipated hours per week
