In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct comprehensive in-home health assessments for members of health plans.

• Carry out approximately 45-minute visits that include reviewing medication, medical history, and performing physical evaluations.

• Execute additional diagnostic tests as requested by the member’s health plan, such as diabetic eye examinations and spirometry tests.

• Foster trusted relationships while working to enhance health outcomes.

• Travel nationwide to visit individuals and link them to necessary care services.

• Travel locally and across licensed states as required.

• Perform virtual consultations when necessary.

• Deliver Signify Health services, which encompass Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screenings.

• Utilize an iPad and follow clinical workflows during evaluations.

• Note that prescribing medication, ordering lab tests, or modifying existing treatment plans is not permitted.

⛳️ Requirements

• Possess an active, unrestricted license(s) in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as required.

• Capability to conduct visits in private homes that may include household pets, secondhand smoke, and various airborne allergens or irritants.

• Proficiency in using an iPad and clinical workflows for evaluations.

• Qualification as a Nurse Practitioner or Physician Assistant is required.
