In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Conduct health evaluations within the homes of members.

• Perform approximately 45-minute appointments.

• Assess medications and medical backgrounds.

• Execute physical assessments.

• Carry out additional diagnostic tests as directed by the health plan.

• Address health inquiries and deliver health education.

• Recognize connections to suitable care and assist in bridging care gaps.

• Travel locally and within authorized states as necessary.

• Facilitate virtual consultations as required.

• Deliver diagnostic and preventive services for Signify Health.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualified as a Nurse Practitioner or Physician Assistant.

• Capability to travel locally and within licensed states as required.

• Ability to conduct home visits that may include household pets, secondhand smoke, and airborne allergens or irritants.

• Proficiency in using an iPad and clinical workflows.

• Competence to provide diagnostic and preventive services, including spirometry, diabetic eye examinations, and bone density screenings.
