In-Home Nurse Practitioner or Physician Assistant, Full Time

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Conduct health evaluations within the homes of members.

• Travel extensively, up to 100% of the time, across a specified coverage area, which may include overnight hotel stays, car rentals, and/or flights.

• Establish trusted relationships to enhance health outcomes.

• Perform visits that last approximately 45 minutes.

• Review members' medications and medical histories.

• Execute physical evaluations.

• Provide additional diagnostic tests as requested by the health plan, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Conduct virtual visits when necessary.

• Deliver Signify Health diagnostic and preventive services.

• Visit health plan members across the nation and link them to suitable care.

• Offer health education and respond to members’ inquiries.

• Work within the private residences of members.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Capability to obtain and maintain multi-state licenses as required in the coverage area.

• Willingness to cover all counties within the state and adapt to other business needs as necessary.

• Comfort and safety in performing duties in homes that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Availability for a commitment of 40 hours per week.

• Readiness and ability to travel locally and within licensed states.

• Competence to conduct virtual visits as required.
