In-Home Health – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Conduct in-home health assessments for members of the health plan.

• Carry out visits lasting approximately 45 minutes that involve personalized health discussions.

• Evaluate medications and review medical histories.

• Execute physical assessments.

• Undertake additional diagnostic tests as directed by the health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflow for each assessment.

• Travel nationwide to connect individuals with suitable care.

• Offer Signify Health services, including Diagnostic & Preventive Services like spirometry, diabetic eye exams, and bone density screenings.

• Conduct virtual consultations as required.

• Travel locally and within licensed states as necessary.

• Establish trustworthy relationships to enhance health outcomes.

⛳️ Requirements

• Possess an active, unrestricted license(s) in applicable coverage area(s).

• Hold board certification (mandatory).

• Multi-state licenses are preferred.

• Capable of safely performing tasks in homes that may have pets, secondhand smoke, and airborne allergens or irritants.

• Willingness to travel locally and within licensed states as necessary.

• Ability to conduct virtual visits when required.

• Qualification as a Nurse Practitioner or Physician Assistant.
