In-Home Health – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct in-home health assessments for members at their residences.

• Interact with individuals to help close care gaps.

• Execute visits lasting approximately 45 minutes.

• Assess medications and review medical histories.

• Carry out physical examinations.

• Perform additional diagnostic tests as directed by the health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Foster trusted relationships to enhance health outcomes.

• Travel across the nation to link individuals with suitable care.

• Travel locally and within licensed states as required.

• Conduct virtual appointments when necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services products.

⛳️ Requirements

• Active and unrestricted license(s) in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as needed.

• Capability to conduct virtual visits when required.

• Ability to safely perform duties in homes that may have pets, secondhand smoke, and other possible airborne allergens or irritants.
