In-Home Health NP/PA – Part Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct in-home health assessments at members’ private residences.

• Complete visits lasting approximately 45 minutes.

• Evaluate medications and review medical histories.

• Execute physical examinations.

• Carry out additional diagnostic tests as prescribed by the health plan, including diabetic eye exams and spirometry.

• Utilize an iPad and follow clinical workflows for each evaluation.

• Travel nationwide to connect individuals with suitable care.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services.

• Facilitate virtual consultations as required.

• Travel locally and within licensed states as necessary.

• Establish trusted relationships and contribute to improved health outcomes.

• Provide education to members and address health inquiries without prescribing medications, ordering lab tests, or modifying current treatment plans.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Capability to work safely and comfortably in homes with pets, secondhand smoke, and potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states as needed.

• Ability to conduct virtual consultations as required.

• Qualification as a Nurse Practitioner or Physician Assistant.
