In-Home Health NP/PA, Full Time

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct in-home health assessments for members of health plans.

• Execute visits lasting approximately 45 minutes, which include reviewing medication and medical history, along with a physical evaluation.

• Carry out additional diagnostic tests as requested by the member’s health plan, such as diabetic eye examinations and spirometry tests.

• Utilize an iPad and clinical workflow for conducting each evaluation.

• Establish trusted relationships and contribute to enhancing health outcomes.

• Travel across the country to connect individuals with suitable care.

• Travel locally and within licensed states as necessary.

• Provide virtual consultations when required.

• Deliver Signify Health services, encompassing Diagnostic & Preventive Services, including spirometry, diabetic eye examinations, and bone density screenings.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as needed.

• Capability to conduct virtual visits as necessary.

• Ability to safely perform duties in homes with pets, secondhand smoke, and other possible airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
