In-Home Health – NP/PA

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct in-home health evaluations lasting approximately 45 minutes.

• Interact with health plan members in their residences to foster trusted relationships.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Perform additional diagnostic tests as requested by the health plan.

• Utilize an iPad along with clinical workflow for each evaluation.

• Travel nationwide to connect individuals with suitable care.

• Travel locally and within authorized states as necessary.

• Conduct virtual consultations as required.

• Offer Signify Health diagnostic and preventive services such as spirometry, diabetic eye examinations, and bone density assessments.

• Educate members and respond to their health-related inquiries.

• Refrain from prescribing medications, ordering laboratory tests, or modifying existing treatment plans.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in applicable coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualifications as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states as necessary.

• Capacity to conduct virtual consultations as required.

• Ability to safely perform duties in homes with pets, secondhand smoke, and potential airborne allergens or irritants.

• Competence in providing diagnostic and preventive services including spirometry, diabetic eye exams, and bone density screenings.
