Hospice Triage RN, PT, Weekend

This is a fully remote position, open to applicants in Florida.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and identify appropriate next steps.

• Collaborate with on-call teams to ensure effective care and resource coordination.

• Document communications and interventions accurately.

• Maintain a composed and professional attitude while managing urgent calls.

• Offer after-hours support and facilitate timely interventions for hospice patients receiving services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Capable of typing documentation and notes in a fast-paced environment.

• Ability to manage stress and multitask effectively while receiving calls.

• Excellent communication and critical thinking abilities.

• Capable of working independently in a remote setting.

• Consistent attendance, proactive communication, and dependable internet connection during scheduled shifts.

• Availability on weekends: Saturday and Sunday, from 7:30 a.m. to 4:00 p.m. CST.

• Starting pay of $27 per hour, with various opportunities to increase the hourly rate, potentially reaching up to $30 per hour within the first 6 months of employment.

• $2 shift differential for overnight shifts.

• $2 shift differential for weekend shifts (Friday evening, Saturday & Sunday).
