Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and decide on the necessary next steps.

• Collaborate with on-call teams to ensure care and resource coordination.

• Accurately record all communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Provide essential after-hours support for the needs of hospice patients and families.

• Facilitate timely interventions and care for patients utilizing hospice services.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license.

• Preference for experience in hospice, palliative, or end-of-life care.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Capacity to type documentation and notes in a fast-paced environment.

• Ability to manage stress and multitask while handling incoming calls.

• Strong communication and critical thinking abilities.

• Capability to work independently in a remote setting.

• Reliable attendance, proactive communication, and stable internet connectivity during scheduled shifts.

• Availability for the specified schedule: 3:30 PM to 11 PM, including rotating Saturday and Sunday shifts from 7:30 AM to 4 PM CST.
