Hospice Triage RN, PT

This is a fully remote position, open to applicants in Texas.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and decide on suitable next steps.

• Collaborate with on-call teams to ensure care and resource coordination.

• Precisely document all communications and interventions.

• Maintain a composed and professional attitude when managing urgent calls.

• Offer essential after-hours support and coordinate care for hospice patients and families via phone.

• Act as an extension of clients’ care teams during non-core hours.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Must be proficient with technology and electronic medical records (EMR) used for call documentation.

• Capability and comfort in typing documentation and notes in a fast-paced setting.

• Must manage stress effectively and multitask while receiving calls.

• Excellent communication and critical thinking abilities.

• Ability to work autonomously in a remote setting.

• Reliable attendance, active communication, and stable internet connection during all scheduled shifts.

• Availability for scheduled shifts from 3:30 PM to 11:00 PM CST, including rotating Saturday and Sunday shifts during the same hours.
