Hospice Triage RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver after-hours triage, clinical evaluations, and interventions over the phone.

• Assist patients and their families with managing symptoms and addressing end-of-life care requirements.

• Facilitate care coordination by working closely with physicians, facilities, and internal teams.

• Organize visits and coordinate services with vendors as necessary.

• Provide clear and empathetic education and support to families.

• Meticulously record all interactions, care plans, and outcomes within the EMR system.

• Assess team documentation for precision and adherence to standards.

• Collaborate with Telecare staff to promote favorable patient outcomes.

⛳️ Requirements

• Current RN license without any existing or pending restrictions.

• Experience in hospice care.

• Familiarity with Home Care Home Base (HCHB) software.

• Strong clinical judgment paired with critical thinking abilities.

• Exceptional communication skills and proficiency in phone-based patient care.

• Capability to manage multiple tasks in a dynamic, remote setting.

• Strong customer service aptitude and conflict resolution skills.
