Hospice Triage RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide triage, clinical assessment, and interventions via phone during after-hours.

• Assist patients and families with managing symptoms and addressing end-of-life care requirements.

• Coordinate care by working collaboratively with physicians, facilities, and internal teams.

• Organize visits and coordinate services with vendors as necessary.

• Offer clear and compassionate education and support to families.

• Document all interactions, care plans, and outcomes accurately in the EMR.

• Review documentation from the team to ensure accuracy and compliance.

• Collaborate with Telecare staff to promote positive patient outcomes.

⛳️ Requirements

• Active RN license with no current or pending restrictions.

• Experience in hospice care.

• Familiarity with Home Care Home Base (HCHB).

• Strong clinical judgment and critical thinking abilities.

• Excellent communication skills and proficiency in phone-based patient care.

• Capability to multitask in a fast-paced, remote setting.

• Strong customer service and conflict resolution abilities.
