Home Health Physical Therapist

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Plan and manage prescribed physical therapy treatments for patients in their residences.

• Evaluate and document patients' physical capabilities to aid physician assessments and inform treatment objectives.

• Design and execute rehabilitation programs aligned with the physician's treatment plan and the goals of the care team.

• Provide written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides.

• Supervise the performance and documentation of PTAs and Home Health Aides.

• Thoroughly and promptly document patient observations, interventions, and evaluations.

• Communicate patient progress to physicians, the Clinical Manager, the care team, patients, and their families.

• Submit evaluations, treatment plans, progress notes, and discharge summaries.

• Assist patients in regaining strength, mobility, and independence through tailored home healthcare services.

⛳️ Requirements

• Possession of a current and unrestricted Physical Therapy license.

• Valid CPR certification.

• Experience in collaborating with a healthcare team.

• A valid state driver's license.

• Maintain personal vehicle liability insurance that meets or exceeds the minimum required limits of the residing state.

• At least one year of experience as a Physical Therapist (preferred).

• TB screening is required upon selection.

• Availability to work 40 scheduled hours per week.
