Home Health Aide – Hospice

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Deliver compassionate, patient-focused personal care to hospice patients in their homes and community environments.

• Support with daily living activities, including bathing, grooming, dressing, toileting, and oral hygiene.

• Assist with safe mobility, transfers, positioning, and ambulation.

• Monitor and promptly communicate any changes in patients' physical conditions, behaviors, comfort levels, or overall wellness to the supervising RN.

• Offer emotional support, companionship, and reassurance to both patients and their families.

• Ensure a clean, safe, and comfortable environment for patients, including light housekeeping tasks related to patient care.

• Adhere to personalized care plans, infection control protocols, safety standards, and hospice policies.

• Relay patient needs, document care provided, and facilitate coordination with the hospice interdisciplinary team.

⛳️ Requirements

• Certification as a Home Health Aide or CNA.

• Possession of a valid Driver's License.

• GED or HiSET, or a high school diploma.

• Basic Life Support certification from the American Heart Association.

• Certification as a Certified Home Health Aide.

• Capability to provide hospice care in patients' homes and community settings.

• Availability for a part-time daytime schedule, totaling 24 hours from 8 a.m. to 4:30 p.m.

• Requirement to work every 8th weekend.
