This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee, organize, and manage both electronic and paper medical records.

• Assess medical records for any deficiencies, missing documentation, signature requirements, and quality of documentation.

• Handle corrections, amendments, and updates to records.

• Assist with the management of designated record sets and the maintenance of legal medical records.

• Conduct routine quality reviews and audits of medical records.

• Process requests for records from patients, caregivers, providers, attorneys, payers, auditors, and regulatory agencies.

• Confirm identity, authorization, and legal requirements before disclosing information.

• Coordinate responses to subpoenas, audits, investigations, and regulatory inquiries.

• Maintain precise disclosure logs and documentation for record releases.

• Ensure adherence to HIPAA, state privacy laws, medical record retention standards, accreditation criteria, and organizational policies.

• Recognize and escalate risks associated with documentation, privacy, security, or record management practices.

• Engage in both internal and external audits along with corrective action initiatives.

• Aid in regulatory readiness and ensure audit preparedness.

• Monitor data quality and documentation standards within electronic medical record systems.

• Provide assistance to departments with documentation inquiries, record requirements, and best practices.

• Identify trends, recurrent deficiencies, and areas for enhancement.

• Facilitate the implementation of documentation standards and workflows.

• Collaborate with Product Management, clinical teams, and operational teams on medical record functionality and enhancements.

• Take part in testing, implementing, and optimizing workflows and technologies related to records.

• Assist with document management, scanning, indexing, and records retention procedures.

• Offer guidance to staff on medical record requirements, release of information protocols, and privacy standards.

• Respond promptly and professionally to both internal and external record requests.

• Foster a culture of compliance, confidentiality, and quality documentation across the organization.


⛳️ Requirements

• High School Diploma/GED is required; a Bachelor's degree is preferred.

• At least two (2) years of experience in health information management, medical records, healthcare compliance, revenue cycle support, or related healthcare operations.

• Experience with electronic health record (EHR/EMR) systems is required.

• Prior experience in processing release of information requests is preferred.

• Experience in supporting audits, accreditation, quality, or payer requirements is preferred.

• Knowledge of HIPAA privacy and security regulations.

• Familiarity with medical record standards, documentation requirements, and principles of health information management.

• Understanding of healthcare regulatory and accreditation mandates.

• Strong attention to detail and accuracy.

• Excellent organizational and time management capabilities.

• Capacity to handle sensitive and confidential information.

• Strong written and verbal communication skills.

• Ability to work independently while balancing multiple priorities.

• Proficiency with Microsoft Office applications and electronic health record systems.

• Capability to analyze records, pinpoint deficiencies, and suggest corrective measures.

• Must effectively communicate within the work environment and be able to read and write in the primary language used in the workplace.

• Visual and auditory abilities to interact continuously with clients, staff, and others in the workplace.

• Frequent speaking and listening (25-75%) to clients, staff, and others in the workplace.

• Utilize computer and cell phone for effective communication.

• Ability to obtain and maintain criminal record clearance through the Department of Justice (DOJ).

• Ability to receive and retain clearance through the Office of Inspector General.

• Must participate in any required training.
