Grant Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as an operational link between IDOH and regional collaborators.

• Provide support to organizations benefiting from the Rural Health Transformation Program by assisting with grant and contract stipulations.

• Oversee compliance with state and federal regulations, allowable expenses, budgets, documentation, milestones, deliverables, and timelines.

• Ensure audit-ready grant documentation and promote consistent application of approved templates, tools, reporting systems, and grants management practices.

• Recognize performance risks, compliance issues, budgetary concerns, and barriers to implementation, escalating matters to IDOH when necessary.

• Work in partnership with statewide Technical Assistance Providers—IHA, IPHCA, and IRHA—to support regional coalitions.

• Engage in regional consortium meetings, workgroups, and planning sessions.

• Organize Regional Committee meetings.

• Facilitate regional data collection, performance monitoring, outcome measurement, and reporting in accordance with CMS Rural Health Transformation Program standards.

• Assist partners in comprehending CMS documentation, metrics, narratives, and reporting expectations.

• Aid in corrective action planning when progress does not meet expectations.

• Function as the daily liaison among IDOH, regional coalitions, healthcare providers, community organizations, and other stakeholders.

• Support partners in interpreting state guidance, addressing implementation challenges, sharing best practices, and utilizing standardized statewide tools and evaluation methods.


⛳️ Requirements

• Bachelor’s degree in public health, health administration, public policy, business administration, or a similar discipline.

• 5 years of experience in grant management, contract oversight, program administration, or related fields.

• 3 years of experience collaborating with healthcare organizations, rural communities, or public health systems.

• 2 years of demonstrated expertise in compliance, budgeting, and reporting requirements for state or federal grants.

• Capability to manage multiple projects concurrently with meticulous attention to detail.

• Outstanding communication, facilitation, and relationship-building abilities.

• Willingness and capacity for regular travel across various regions of Indiana.

• Experience with CMS-funded programs or federal cooperative agreements is preferred.

• Familiarity with Indiana’s rural health environment, including hospitals, FQHCs, EMS, and community-based organizations is preferred.

• Experience collaborating with statewide associations or multi-stakeholder coalitions is preferred.

• Knowledge of rural health delivery, care coordination, or health system transformation is preferred.
