This is a fully remote position, open to applicants in New Mexico.
• Reach out to members and community partners to enhance healthcare initiatives and activities.
• Implement educational and quality campaigns.
• Assist in organizing appointments for care managers and members.
• Carry out ongoing tasks related to members' health-related social needs, either directly or through social service departments and community organizations.
• Address member inquiries.
• Aid clinicians in managing physical and behavioral health care through provider and member activities.
• Foster relationships with community partners and collaborate with community-based organizations.
• Contribute to the development of community health outreach programs.
• Engage in fieldwork as necessary for initiatives, health campaigns, and departmental responsibilities.
• Oversee low-risk members regarding their health-related social needs, education, basic outreach screenings, assessments, and inquiries.
• Bachelor's degree OR 1 year of college plus 3 years of experience in health, social service, or community work OR 4 years of experience in health, social services, or community settings.
• Familiarity with medical terminology.
• Experience in coordinating member-related needs, assisting members, and analyzing their requirements.
• Proficient in PC use, including Microsoft Office applications.
• Strong customer service abilities.
• Excellent verbal and written communication skills, including the capacity to create written correspondence for members and departmental personnel.
• Coaching capabilities, including motivational interviewing, to educate members on medical and behavioral health topics.
• New Mexico only: Bachelor's degree in Social Work.
• New Mexico only: Proficiency in a New Mexico Native American Pueblo language; must be able to translate and present effectively.
• New Mexico only: Community Health Worker (CHW) certification.
• Preferred: Experience in community engagement.
• Preferred: Background in supporting populations with health-related social needs.
• Candidate must be located in the Las Cruces, New Mexico area.
• Remote work is allowed within the continental United States, excluding California, New York, Alaska, and Hawaii.
