Float Case Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members while identifying their strengths, interests, and preferences.

• Create detailed Member Care Plans (MCPs).

• Collaborate with a Field Care Nurse to deliver services that meet members’ health and safety needs.

• Perform in-person social assessments at enrollment and at least every six months thereafter.

• Conduct quarterly face-to-face member visits and maintain monthly telephone communication.

• Organize support services related to social integration, community resources, employment, housing, and other non-medical necessities.

• Ensure delivery of services in a cost-effective manner.

• Assess risk factors and provide education to members.

• Keep accurate case notes, service authorizations, and updates to MCPs.

• Serve as a float to cover caseloads for team members in the SW Pod 2 region who have left their positions or are on leave.

⛳️ Requirements

• A four-year bachelor’s degree in Human Services or a related discipline, along with one year of experience working with at least one Family Care target population, OR a four-year degree in a different field with three years of relevant experience in at least one Family Care target population.

• A minimum of three years of experience in Family Care.

• Must reside within the SW region of Wisconsin.

• Must be willing to travel up to 40% within the SW region.

• Possession of a valid state driver’s license.

• Proof of personal vehicle liability insurance with minimum coverage limits of 100/300/100.

• Ability to comply with Humana’s patient-facing TB screening requirement.

• Access to home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.

• A dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information.
