Case Manager – Vocational and Medical Rehabilitation

This is a fully remote position, open to applicants in Germany.

📋 Description

• Enhance and clarify the medical care requirements and positive functional capacity profile for clients in rehabilitation.

• Offer vocational counseling and support for job placement to rehabilitation clients.

• Administer profiling and training for job applications.

• Identify job opportunities through tailored educational support.

• Record counseling and placement activities in written format.

• Act as a person-centered navigator, systematically coordinating services and support.

• Organize case-related activities with funding agencies and service providers in the regional network.

• Collaborate with rehabilitation facilities, social benefit providers, counseling centers, and self-help organizations.

⛳️ Requirements

• A completed degree in a humanities or social science field, such as social work, education, or psychology, along with a minimum of two years of professional experience in vocational (re)orientation counseling.

• A completed psychology degree must also include evidence of at least 40 teaching units in social welfare law.

• Alternatively, a completed vocational training in the health and social care sector or additional training in rehabilitation, each requiring at least five years of professional experience in vocational (re)orientation counseling.

• Verification of methodological and social welfare law knowledge: 160 teaching units in counseling skills and 40 teaching units in social welfare law.

• Strong customer orientation, exceptional motivational abilities, empathy, and persuasiveness.

• Preferred: Case Management training or CDMP qualification.

• Familiarity with the structures of social benefit providers and rehabilitation law.

• Experience in vocational and medical rehabilitation, along with solution-focused case management.

• Proficient in MS Office, particularly Microsoft Word, and internet applications.

• Willingness to travel locally, ideally with personal vehicle.
