Field Care Coordinator – Bilingual Preferred

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct home visits for Medicaid members, including those in Assisted Living Facilities, Long Term Care Facilities, and other care environments.

• Travel locally 75–90% of the time.

• Assess and evaluate the needs and requirements of members.

• Develop care plans tailored to individual members.

• Ensure that members receive services in the least restrictive settings possible.

• Plan and execute interventions as needed.

• Coordinate services and monitor case management plans, evaluating them against members' personal goals.

• Guide members and their families toward suitable care resources.

• Facilitate communication with payer sources, providers, interdisciplinary teams, and others involved in the care of members.

• Report to the Regional Care Coach Manager.


⛳️ Requirements

• Bachelor's degree in a field related to health and human services.

• At least 2 years of relevant experience.

• Proficiency in Microsoft Office Word and Excel.

• Outstanding verbal, written communication, and interpersonal skills.

• Capability to utilize various electronic information applications and software programs.

• Must be able to pass tuberculosis (TB) screening.

• A strong commitment to enhancing consumer experiences continuously.

• Valid driver's license from the state.

• Maintain personal vehicle liability insurance that meets state minimums or specified higher limits.

• Must reside in Hillsborough or Hardee counties in Florida.

• Must have a designated room with a locked door suitable for a private home office.

• Previous experience with electronic case note documentation is preferred.

• Prior experience as a Medicare and Medicaid recipient is preferred.

• Experience in health promotion, coaching, and wellness is preferred.

• Familiarity with community health and social service agencies, as well as community resources, is preferred.
