Field Care Coordinator

This is a fully remote position, open to applicants in Florida.

📋 Description

• Evaluate and assess the needs and requirements of Medicaid members.

• Create individualized care plans for members.

• Conduct visits to members in their homes, assisted living facilities, long-term care facilities, and other care environments.

• Travel locally 75-90% of the time.

• Ensure that members receive services in the least restrictive environment possible.

• Plan and execute necessary interventions.

• Coordinate services and monitor as well as evaluate case management plans in relation to members' personal goals.

• Assist members and their families in navigating appropriate care resources.

• Facilitate communication with payers, providers, interdisciplinary teams, and other parties involved in the care of members.

• Provide health education, motivation, and support.

• Connect members with high-quality services to enhance well-being and improve health outcomes.

⛳️ Requirements

• Bachelor's degree with a minimum of 2 years of relevant experience in Health Services, Social Work, or Psychology; OR Registered Nurse (RN) licensed in Florida with at least 2 years of relevant experience; OR Licensed Practical Nurse (LPN) licensed in Florida with a minimum of 4 years of relevant experience; OR Case Worker with at least 6 years of experience in Health Services/Case Management.

• Prior experience working with Medicare & Medicaid recipients.

• Must reside in Lake County, Florida.

• Intermediate to advanced proficiency in computer skills.

• Familiarity with Microsoft Word, Excel, and Outlook.

• AHCA Fingerprint clearance is required.

• Capability to complete the necessary TB screening.

• Valid driver's license for the state.

• Proof of personal vehicle liability insurance that meets minimum standards.

• Availability to work Monday through Friday from 8:30 am to 5:00 pm.

• Minimum home internet speed of 25 Mbps for downloads and 10 Mbps for uploads for employees working from home or in a hybrid setting.

• A dedicated workspace free from interruptions to safeguard member PHI/HIPAA information.
