Family Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct virtual initial appointments, encompassing new patient intake, health history assessments, baseline evaluations, and an introduction to the PeopleOne Health model.

• Provide remote virtual urgent care for acute conditions, minor injuries, medication requirements, and symptom assessments.

• Triage patients to in-person care, diagnostics, or specialists as needed.

• Educate and guide patients on self-care, medication usage, and follow-up requirements.

• Ensure accurate, timely, and compliant EHR documentation for virtual interactions.

• Collaborate with primary care providers, clinical pharmacists, registered dietitians, health coaches, and care coordinators.

• Act as a welcoming first point of contact and promote a positive member experience.

• Work alongside Client Success and Member Experience teams to enhance patient engagement.

⛳️ Requirements

• Master’s Degree in Nursing (MSN) or Doctor of Nursing Practice (DNP).

• Current and unrestricted Nurse Practitioner license.

• National Board Certification in Family Medicine (FNP).

• DEA Certificate (as required).

• A minimum of 2+ years of practice in Family Medicine or Urgent Care.

• Telehealth experience is highly preferred.

• Experience in value-based or team-based care models is advantageous.

• Strong skills in telehealth-based assessment and triage.

• Capacity to work efficiently in a high-access, episodic care model.

• Exceptional communication and patient education abilities.

• Proficiency with technology, telemedicine platforms, and EHRs.

• Reliable internet connection and a private workspace.

• Adherence to HIPAA standards.

• Availability to work until 8pm ET on two evenings per week.

• May be required to work evenings or weekends to fulfill urgent care access needs.
