This is a fully remote position, open to applicants in Arizona, +10 more states.
• Perform targeted telephonic outreach to patients, families, and healthcare organizations.
• Provide education to patients and families regarding palliative care, VyncaCare services, and program expectations.
• Conduct structured eligibility screenings that include demographics, chronic disease history, and functional decline.
• Verify service-area eligibility.
• Schedule initial visits for eligible patients with nurse practitioners.
• Manage enrollment logistics and ensure smooth transitions from enrollment to care.
• Act as a liaison between patients and the wider care team.
• Direct calls, texts, and emails to the relevant individuals or departments.
• Address patient inquiries and concerns promptly and supportively.
• Keep patient documentation and communications confidential and accurate, in compliance with HIPAA and organizational standards.
• Coordinate enrollment activities across targeted regions and monitor outreach efforts.
• Identify trends in outreach and barriers to enrollment.
• Share both qualitative and quantitative feedback, along with innovative ideas, with the market and business development team.
• Maintain collaborative relationships with the designated market team and practice leadership.
• Contribute to program expansion, growth in census, and improved patient access.
• Current CMA or CCMA certification from a recognized credentialing organization.
• Minimum of 2 years of healthcare experience required.
• 8 months of prior clinical experience preferred.
• 1 year of experience in telephonic or call center customer service.
• Experience in a medical practice or healthcare environment.
• Familiarity with EHR platforms for documenting patient interactions and scheduling appointments.
• Training and experience in motivational interviewing and patient education.
• Comfort discussing serious illness, chronic disease, and end-of-life topics with patients and families.
• Strong organizational capabilities and the ability to work independently while managing a high-volume outreach workload.
• Proficient in Microsoft and Google applications.
• Access to a dependable internet connection.
• Excellent written and verbal communication skills.
• Outstanding interpersonal abilities, including conflict resolution and de-escalation techniques.
• Willingness and capability to work Monday through Friday, 8:30 am – 5 pm Eastern Standard Time, with the possibility of overtime.
• Bilingual in English and Spanish preferred.
• Certification in medical terminology preferred.
• Familiarity with palliative care, home-based care models, or chronic disease management preferred.
