This is a fully remote position, open to applicants in Pennsylvania.
β’ Provide nutrition care to patients by conducting assessments, planning, implementing, and monitoring their nutritional needs.
β’ Conduct personalized patient assessments and reassessments.
β’ Create, document, and oversee nutritional care plans based on individual assessments and professional standards.
β’ Deliver nutrition education and develop educational programs for both patients and staff.
β’ Complete documentation and reports in accordance with established procedures and practices.
β’ Design, implement, and evaluate tools to enhance and sustain nutrition care for patients, staff, and the community.
β’ Carry out other assigned or necessary duties as required.
β’ Adhere to HIPAA, privacy, information security, legal, ethical, and company policy standards.
β’ A minimum of a Bachelor's Degree in Dietetics or Nutrition.
β’ Relevant experience within a specialized practice.
β’ Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD), or eligibility for registration with required attainment within six months of hire.
β’ Licensed Dietitian/Nutritionist (LDN), or eligibility for licensure with required attainment within nine months of hire.
β’ Act 34 Criminal Background Clearance Certificate.
β’ Act 33 Child Abuse Clearance Certificate.
β’ Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
