This is a fully remote position, open to applicants in Illinois.
• Collaborate individually with members to develop customized care plans and coordinate necessary services.
• Evaluate members' goals, needs, obstacles, resources, and their ability to reintegrate into the community.
• Offer specialized assistance to members receiving Long Term Services and Support (LTSS).
• Address health-related social concerns and provide psychosocial assistance.
• Support members living in nursing facilities by fostering relationships with facility staff and advocating for their care.
• Engage with members both over the phone and in person to set goals, assess resources, create care plans, and identify providers and community partnerships.
• Assist members and their caregivers in accessing LTSS, as well as social, housing, educational, and other essential services.
• Create and adjust Individual Service Plans while involving relevant members of the care team.
• Navigate LTSS and associated environmental and social requirements.
• Utilize available member data to avoid repetitive assessments.
• Suggest Home and Community-Based Services (HCBS) to promote member independence.
• Facilitate communication with payers, providers, and interdisciplinary care teams.
• Educate members on how to maintain their Medicaid eligibility.
• Input annual Level of Care assessments into the state portal.
• Travel 75–90% of the time in the field to visit members.
• Must be a resident of the state of Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related discipline.
• A minimum of two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), or managed care organizations.
• At least one (1) year of professional experience working with members in custodial and transitional care settings.
• Proficiency in using electronic information applications/software programs, including electronic medical records.
• Willingness to travel within the region to meet in person with members, families, community partners, and care teams.
• Valid state driver's license required.
• Personal vehicle liability insurance that meets the required limits.
• Availability to work Monday–Friday, from 8:00 AM to 5:00 PM Central Standard Time, with some flexibility.
• Ability to complete TB screening if selected for the position.
