Complex Care Registered Nurse – Bilingual English/Spanish Preferred

This is a fully remote position, open to applicants in California.

📋 Description

• Take charge of the Member Journey and Care Coordination for High-Acuity Members.

• Act as the main care coordinator for a designated group of medically intricate, high-risk Medicare Advantage members.

• Oversee Transitions of Care and Hospital Discharge Coordination.

• Conduct Medication Reconciliations and Clinical Monitoring.

• Elevate Clinical Concerns and Assist APC Clinical Decision-Making.

• Enable Cross-Disciplinary Care Coordination Within the Pod.

• Contribute to HEDIS, Quality, and Care Gap Closure initiatives.

• Administer Chronic Condition Care Pathways and Guideline-Directed Care Delivery.

• Record All Clinical Interactions Accurately and Promptly in Athena.

⛳️ Requirements

• At least 3 years of clinical RN experience.

• Active and unrestricted Registered Nurse (RN) license in relevant state(s).

• Proven experience in managing medically intricate, high-risk patient populations.

• Familiarity with telehealth or virtual care delivery models.

• Associate Degree in Nursing (ADN) — Bachelor of Science in Nursing (BSN) is highly preferred.

• Current BLS certification.
