Clinician Field Mentor

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct In-home Evaluations, Focused Visits, and Video Visits

• Demonstrate Signify Health's operational, clinical, and member experience key performance indicators (KPIs)

• Complete up to 80% of the workweek in visits, reserving one assigned day weekly for coaching and mentorship

• Ensure 40 hours of weekly availability, achieve 128+ capacity units monthly, and complete 88+ visits each month

• Offer dedicated support to new hire clinicians during their initial 90 days

• Facilitate both virtual and in-person shadowing opportunities

• Provide best-practice guidance on clinical workflows and diagnostic and preventive services

• Deliver group support to active clinicians through webinars and office hours

• Offer real-time assistance to clinicians in the field

• Act as the primary response point for clinical inquiries and escalate issues to Clinical Leaders as necessary

• Test new diagnostic and preventive services and changes in clinical workflows, providing constructive feedback

• Meet regularly with Clinical Leaders for ongoing support and training

• Participate in manager, team, clinical, and special-event meetings as required

• Provide rotating mentorship and coaching support during evenings and weekends

• Maintain necessary supplies and diagnostic equipment

• Report to the Operations Manager while having a dotted line to the Clinical Success team

⛳️ Requirements

• Clinical background with state licensure (e.g., MD/DO, NP/PA)

• At least 6 months to 1+ year(s) of experience as a high-performing clinician at Signify Health

• Preferred: multi-state licensure

• Preferred: 5–7 years of clinical experience, particularly in primary care, chronic disease management, or a related field

• Proven experience in successfully completing In-home Evaluations in line with Signify Health's operational, clinical, and member experience KPIs

• Experience in mentoring teams and providing individualized professional and personal support

• Familiarity with Signify In-home Evaluation tools and processes

• Experience in developing and delivering group presentations

• Ability to inspire, motivate, and mentor colleagues

• Capability to share best practices, tips, and insights from direct experiences

• Understanding of managing inquiries and escalating performance issues to leadership

• Knowledge of maintaining high-quality member care across teams

• Physical capability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies

• Availability to provide 40 hours of service weekly

• Willingness to travel locally and regionally as necessitated by business requirements

• Ability to accommodate field observations and ride-along experiences

• Commitment to maintaining training, affiliation, and compliance with Video Visit requirements

• Dedication to completing all diagnostic and preventive service products as instructed
