This is a fully remote position, open to applicants in Michigan.
• Support the triage process for incoming sick calls from patients of the Medical Group.
• Evaluate patient symptoms and deliver suitable medical guidance.
• Ascertain the necessary level of care and collaborate with the care team to coordinate services.
• Analyze clinical test results and provide summaries to assist healthcare providers.
• Prioritize and respond promptly to incoming phone calls, voicemails, and test outcomes.
• Conduct phone assessments utilizing probing inquiries and the EMR system.
• Implement evidence-based guidelines to establish triage priorities.
• Recognize crisis situations and link patients with emergency services as needed.
• Educate patients and caregivers on health maintenance, disease prevention, self-care practices, medications, and follow-up actions.
• Make outbound calls to relay provider directives.
• Work collaboratively with care teams regarding patient needs and follow-up procedures.
• Engage with patients and their families, offering emotional support.
• Analyze data results and share findings with healthcare providers.
• Enter patient outcomes and quality metrics into the system.
• Address and escalate patient complaints, grievances, and concerns effectively.
• Achieve quality and productivity benchmarks.
• Involve in projects and initiatives aimed at quality improvement.
• Carry out additional responsibilities as assigned by the Clinical Triage Manager and/or Director.
• Graduate from an accredited nursing education program as a Registered Nurse.
• Possess a current and valid RN license.
• Must obtain and maintain RN licensure across all service states within six months of hire.
• Successfully complete required continuing education for nursing.
• Ability to react quickly and decisively in urgent and emergency situations.
• Strong understanding of medical terminology and pharmaceuticals.
• Proven experience in multitasking effectively.
• Excellent communication skills, both written and verbal.
• Demonstrated proficiency in using electronic health records (EHR) systems.
• Capable of effective communication with providers, patient care teams, patients, their family members, and facility staff.
• Ability to work independently and remotely.
• Knowledge of fundamental principles, practices, and techniques in primary healthcare.
• Strong organizational skills and meticulous attention to detail.
• Conflict resolution abilities.
• Familiarity with Medicare and Medicaid insurances and programs.
• Over 2 years of experience as a Registered Nurse.
• Experience in a contact center setting.
• Background in nurse triage.
• Experience with Aprima (EHR).
• Multistate License (MSL) is required.
